Our Community Health Needs Assessment Process
Upon engagement of AscentHealth Consulting, we immediately create an Assessment Plan and begin constructing the instrument to be used. We build a database of organizations and thought leaders to be surveyed throughout the process and work to engage gatekeepers and champions for community health.
We provide timelines and milestone reports as we move through the process. We work to provide the best in a community health assessment to meet your needs — in a way that fits each organization’s unique situation.
Our Findings and Recommendations Report
After completion of the Community Health Needs Assessment, we present the Findings and Recommendations Report (FRR) in a printed, bound format along with a PowerPoint file for public presentation. Our FRR provides a comprehensive analysis of the trends noted in the primary and secondary data sets.
We give those results in a concise, cogent report with simple descriptions, graphical displays and charts to illustrate the findings. After thorough reporting of those health trend results, we provide our recommendations for addressing the issues outlined in the results. These recommendations are limited to the most pressing challenges and will be prioritized by our expert staff. These recommendations will provide focus and give direction for organizations, community members and policy makers to move the community’s health forward.
To facilitate these discussions, we are available for formal presentation to groups as directed by our clients. Our professional presenters can facilitate an interactive, town hall meeting style or perform a more didactic analysis in an academic fashion — we will approach the delivery of the report in the way that best fits the client’s needs.
